2. Documents required for all clinical experiences. Please print, complete, and submit to the Hospital Human Resources Department prior to the start of the clinical rotation the following documents.

  2. Student Welcome Orientation Test and Answer Sheet
  3. HIPAA Test and Answer Sheet
  4. Compliance acknowledgement
  5. SIH Standards of Performance & signature page
  6. Emergency Contact form
  7. Confidentiality Agreement
  8. Internet Access Agreement

(Note: The test(s) must be completed and submitted to the Hospital Human Resources Department between Monday-Wednesday, 8:00 am-4:00 pm prior to the first clinical day.)
