The SIH Foundation invites you to create your lasting legacy with a future gift (also called a planned gift) that will help us build a healthier Southern Illinois.
By including the SIH Foundation in your will or trust, or by beneficiary designation in a retirement plan or life insurance policy, you can pass on your passion of caring for others to future generations. Your gift costs you nothing now, but it gives you the power to change lives by providing financial assistance to those in need and enhancing facilities that provide vital healthcare services in our community.

Sample Bequest Language: “I (name), of (city, state, ZIP), give devise and bequeath to the SIH Foundation (written amount or percentage of estate of description of property) for its unrestricted use and purpose.”
Become a Member of our Legacy Society
Those who make a commitment to improving the health and well-being of our communities for future generations through a planned gift are invited to be members of the SIH Foundation Legacy Society. You will have the option to be recognized on donor walls at SIH Hospitals and the SIH Cancer Institute, as well as in SIH publications.
If you decide to include the SIH Foundation in your planning, please complete the Statement of Intent. If you have already included us in your planning, thank you! Please let us know your intentions so we may plan for your gift in accordance with your wishes, and properly acknowledge your generosity.
