Power of Attorney Appointments for Second Act Members

Second Act Members:

We are offering free 30-minute appointments on November 19 between 9 a.m. and 11 a.m. to help you complete your Power of Attorney for healthcare. You’ll get one-on-one support and leave with your documents in hand — no cost, no hassle.

Appointments will take place at the SIH Community Classroom (enter via SIH Outpatient Rehabilitation), located at 1241 E. Main, Carbondale.

Appointments are required, so please call 877.480.4040 to reserve your time. Kindly note that we are not accepting reservations or registrations online—phone calls are the only way to book.
