If your organization is approved:
- An intake form will be emailed to the contact identified on the questionnaire on this page.
- When the intake form is received, a Docusign electronic EpicCare Link Site Level Agreement is emailed to the President/CEO/Owner of the organization.
Note: The signing of the Agreement must be done electronically via Docusign. Do not print off Agreement, wet sign, and email back a .pdf version. - When the Agreement is electronically signed by all parties, a password agreement and user request document is emailed to your organization's site level administrator.
Note: The password agreement and user access request must be signed by the President/CEO/Owner of the organization. - When the password agreement and user access request is received, SIH begins the user access process.
Note: Due to the age of our IT user account identity management information system, a user's access may take two to three weeks. A new system is scheduled to be implemented in October 2024, and user access turn-around time will be decreased. - Username and password credentials are emailed to the user.
Other Important Information
- Organizations must identify a Site Level Administrator.
- The Site Level Administrator holds the responsibility for overseeing access management, which includes processing new access requests, implementing modifications for existing users, and executing access deactivation when necessary.
- Individuals who have not accessed EpicCare Link in 90 days are automatically deactivated.
- A users' misuse of their access results in permanent deactivation and potentially a termination of the organization's EpicCare Link Site Level Agreement.
