Advanced Care Planning helps patients and families make informed decisions about future healthcare needs. Managed through Primary Care and supported by Social Work and Palliative Care teams, this service guides individuals in documenting preferences, ensuring their values and wishes are respected.
Bone Mass Measurements are essential for assessing bone density and identifying conditions like osteoporosis. This service, often managed through our Primary Care service line, helps detect early signs of bone loss, allowing patients to take proactive steps to maintain bone health. Early detection can prevent fractures, improve mobility, and enhance overall quality of life, especially for older adults or individuals with risk factors for bone conditions.
Cervical Cancer Screenings are a vital preventative measure provided by your primary care provider or OB/GYN. Pap tests and HPV screenings can detect abnormal cell changes before they develop into cancer. Early intervention significantly improves outcomes, empowering women with the knowledge and care they need to protect their reproductive health.
Cholesterol Screening, part of our Primary Care services, evaluates levels of LDL (bad cholesterol) and HDL (good cholesterol). Monitoring cholesterol is key to preventing heart disease and stroke. This screening enables patients to make informed lifestyle changes or seek treatment to manage their cardiovascular health.
Diabetes Screening identifies early signs of prediabetes and diabetes, enabling patients to take control of their health before complications arise. This service, integrated into our Primary Care and Endocrinology offerings, is essential for individuals with risk factors such as family history or obesity. Early detection allows for lifestyle changes and management plans to prevent or delay the onset of diabetes.

This program is ordered by your Primary Care Provider or Endocrinologist and covers essential topics such as blood sugar monitoring, medication use, nutrition, and exercise, improving quality of life and reducing the risk of complications like nerve damage or kidney disease.
This is an individualized program to help you manage your diabetes. Your insurance will be billed.
Diabetic Retinopathy Screening is performed by an Optometrist or Ophthalmologist and requires a referral from your Primary Care Provider or Endocrinologist. This critical screening detects early signs of vision loss due to diabetes, allowing for timely treatment to prevent blindness. Regular screenings are vital for patients with diabetes to maintain their eye health.
Hypertension and Blood Pressure Screenings are fundamental for heart health. High blood pressure often has no symptoms but can lead to severe conditions like heart attacks or strokes if left untreated. Regular screenings through your Primary Care Provider help patients monitor their health and implement necessary interventions.
Immunizations are a cornerstone of public health, offering protection against serious diseases such as flu, pneumonia, and COVID-19. Available across Primary Care and Urgent Care clinics, these vaccines not only safeguard individual health but also protect the community by reducing the spread of infectious diseases.
Lung Cancer Screenings utilize low-dose CT scans to detect lung cancer in its early stages, especially for individuals with a history of smoking. Early detection significantly improves treatment outcomes and increases survival rates, making this screening a critical tool in cancer prevention. Your primary care provider, Pulmonologist or Oncologist can order a Lung Cancer Screening.
Medication Compliance support ensures patients follow prescribed treatment plans, improving outcomes for chronic conditions such as diabetes, hypertension, or asthma. Our teams collaborate to provide education and tools to enhance adherence, reducing hospitalizations and improving health outcomes.
Obesity Screening, offered through our Weight Management services, helps identify individuals at risk for conditions such as diabetes, heart disease, and joint problems. Early intervention through tailored diet, exercise, and counseling programs can lead to long-term health improvements and reduced chronic disease risk.
Prostate Cancer Screenings are offered through our Cancer Care service lines, focusing on early detection of one of the most common cancers among men. Regular screenings, including PSA tests, help identify cancer early when it is most treatable, giving patients the best chance for successful outcomes.
