Thank you for considering SIH for your student rotation experience.
Clinical student rotations are defined as experiences that are coordinated through a student's current training program and involve direct patient care. Participation earns a student credit towards their degree or certification. We want to support our local students, schools, and training programs and hope to keep the best students from the region working here in southern Illinois. Read all the information below, and let's get started!

What is clearance to wear a respirator and how do I meet this requirement?
Clearance to wear a respirator is completed by answering a series of heath questions recommended by OSHA (Occupational Health and Safety Administration). If there are positive responses to questions, then a medical review and possibly an exam is required to ensure safe use of a respirator. Once medically cleared, training and fit testing is required to ensure proper PPE is used appropriately and fits the user. Some people have medical conditions that could restrict their ability to breath effectively wearing a respirator. Other conditions could hinder the appropriate fit and use of the respirator.
Please submit this completed OSHA questionnaire along with the required documentation listed in Step 2 to students@sih.net.
